Online Registration for Faith Formation
begins August 1, 2023 and continues through August 31.

Click HERE to print the Authorization Form
to specify how images of your child may or
may not be used by the parish.
When you have completed the authorization form,
please return it along with your payment
to Teresa Frederick in one of these ways:
| 1) Drop off at the school office |
| 2) Put in an envelope marked "Teresa Frederick - Faith Formation" and put in the collection basket at mass. |

| 4) Email For those who have the ability to scan the completed and signed document, you may email it to Teresa Frederick at tfrederick@icclarion.org You will need to get your payment to her in one of the other ways listed above. |
