The Authorization Form is still available HERE to specify how images of
your child may or may not be used by the parish.
If you need to complete a new form, please return it to Teresa Frederick in one of these ways:

•  Drop off at the school office
• Put in an envelope marked "Teresa Frederick - Faith Formation" and put in the collection basket at mass
• Mail to:
