Online Registration for Faith Formation is closed.
Please contact Teresa Frederick if you need to register your child.

to specify how images of your child may or
may not be used by the parish.
When you have completed the form, please return it
to Teresa Frederick in one of these ways:

| 2) Put in an envelope marked "Teresa Frederick - Faith Formation" and put in the collection basket at mass. |
